Abstract

BACKGROUND: Epidural injections of glucocorticoids are commonly given to relieve low back pain and sciatica and improve mobility without surgery. The main aim of this study is to evaluate the analgesic effects of caudal epidural steroid injection in management of chronic low back pain and sciatica. OBJECTIVE: A double blinded prospective and controlled study carried out in our institute after getting approval from ethical committee, to evaluate the analgesic effects of caudal epidural steroids in management of chronic low back pain and sciatica. METHOD: A total of 60 patients of both sexes aged 18 to 80 years of ASA PS 1 and 2, with low back pain and sciatica due to herniated disc included in this study. Visual analogue score and straight leg raising test assessed before procedure. Drugs triamcinolone acetate 80 mg 2ml and bupivacaine 0.125% 8ml were given through ultrasound guided caudal epidural injection Visual analogue score, heart rate mean arterial pressure and saturation assessed for 24hrs.Also Visual analogue score, SLRT and ODI score assessed at 1week, 3mnth and 6month.At the end of study, data was subjected to statistical analysis. RESULTS: A total of 60 patients were enrolled and completed the study. Demographic data of these patients is presented .Pain relief was primary index for evaluating outcome of study. We found that intervention group had large number of patients who reported complete pain relief at the end of 6month evaluation period. The mean VAS score and Oswestry disability score kept decreasing and was statistically significantly lower compared with the score before injection. CONCLUSION: Ultrasound guided caudal epidural steroid offers relatively simple and easily performed day care procedure that can offer significant pain relief and an alternative approach in managing low back pain and sciatica.
