Forty five cases with Vathatambam, was diagnosed clinically. Out of them Fifteen cases were admitted as in-patient PG Sirappu Maurthuvam Ward, Govt. Siddha Medical College Hospital, Palayamkottai were observed for clinical diagnosis, lab diagnosis and treatment by the trial medicines. Out of them five were treated with internal medicine alone. Five were treated with thokkanam therapy alone and remaining five were treated with varma therapy alone. Thirty cases were treated as outpatients. Out of them ten were treated with both external and internal medicine. Ten were treated with internal medicine only and the remaining ten were treated with Thokkanam therapy alone. Clinical diagnosis of Vathatambam was done on the basis of clinical features described in Yugi vathiya sinthamani Manuscript, and siddha maruthuvam text. 1. Before starting the treatment, careful detailed history was taken out and recorded for the 45 selected cases only. 2. The various Siddha aspects of examination of the disease were carried out and data were recorded in the proforma. 3. Laboratory diagnosis of Vathathambam was done by modern methods of examination in the Govt. Siddha Medical College Hospital, Palayamkottai. 4. The trial medicine choosen for both internal and external treatment and the management of Vathathambam. • Maga veera mezhuku normal dose 50mg thrice a day with palmjakkery (Internally). • Ulunthu thaylam (Externally). 5. Before starting treatment, mild purgative was given to all cases. 6. The observation made during the clinical study shows that the main drug Magaveera mezhuku (Internally) is clinically effective. It has significant Analgesic action and Significant of Acute and Chronic Anti Inflammatory Action. 7. The action of Ulunthu thaylam (Externally) over the lumbar region was also clinically effective. It has Significant Analgesic action and Significant Acute - Anti Inflammatory Action. 8. Internal medicine indicates the presences of calcium, Chloride, starch, ferric iron, ferrous iron,un saturated compounds, Reducing sugar and mercury. All these Bio- chemical products are much important for Intervertebral disc repair and its maintenance. 9. A periodical laboratory investigation were made for all the cases for blood, urine and motion test etc., along with radiological reports. 10. Since Vathathambam is a chronic disease, it required minimum treatment for twenty days in inpatients ward. And then patient was advised to continue the treatment as out patient for some period as per their requirement. 11. Though there was appreciable clinical improvement, there was not much remarkable MRI changes. 12. During the period of treatment, all the patients were put under strict pathiyam – a specific dietary regimen. 13. The advantages of the selected drugs were listed as, • The drugs were found to be free from adverse effects. • The raw materials were available in almost all seasons. These merits were essential in promoting this drug in future globally. CONCLUSION: The outcome of this study is that the Maga veera mezhuku and the Ulunthu thylam has moderate effect in the cases of Vathathambam. It was noted that the internal drug Maga veera mezhuku was free from adverse side effects i.e. no cases were reported either nausea, vomiting, gastric irritation, other GI disturbance and allergic manifestations. External application Ulunthu thylam was not irritant i.e no cases were reported itching or eruption whenever applied.
